Nurse's Corner

May Health Reminders:

  • Kindergarten, 2nd and 6th grade parents: A dental exam is due by May 15th
  • If your child is on/has any medication at school, they must be picked up by the end of the school year (May 28th-Which is a half day of school), or it will be disposed of. Absolutely no medication will be sent home with students and will not be stored over summer break. There is a detailed letter being sent home with your child if they have medicine at school. Please be looking for it! All forms must be updated each school year.


IMPORTANT Reminders for next school year: All students are required to have the following health records on file in order to stay in school. These are mandated regulations from the state, and must be turned in or child will be excluded from school if they are not completed. It is recommended to complete all health requirements as soon as possible in order to avoid summer rush and possibility of exclusion from school, which will occur on October 15, 2020. If health requirements are not met by October 15, 2020, your child will not be allowed to attend school until they have been met. All forms can be found under district health requirements. Your doctors' office should have a copy as well.

  • Pre-School: A school physical along with these required vaccinations: 4 DTaP, 3 IPV, at least 1 HIB, pneumococcal, Varicella and MMR vaccines, and a completed Hepatitis B Series. Also, please make sure the parent portion on the back of the physical is completed.
  • Kindergarten/New Student(s): A school physical with a lead screening along with these required vaccinations: 4 or more DTaP with the last dose after age four, 3 or more IPV-with the last dose after age four, 2 Varicella and MMR vaccines; A vision and dental exam. Also, please make sure the parent portion on the back of the physical is completed.
  • Second Grade:A dental exam.
  • Sixth GradeA school physical along with these updated vaccinations: Tdap booster, Hepatitis B Series completed, 2 Varicella, 2 MMR and a Meningococcal vaccine; a dental exam. Also, please make sure the parent portion on the back of the physical is completed.
